Health Inspection Report Week of 1/07/2025

The Inspection Services Department carries out the general health & safety inspections of occupancies requiring a health permit within the jurisdiction of the City of Nacogdoches. The primary function of the Health Inspector is the interpretation, inspection and field compliance efforts of the technical codes and laws. These codes and laws are Federal, State and Local. In addition the Health Inspector with support from the Inspection Services Department carries out the responsibility of data entry, account billing and record keeping.

Weekly Health Inspections: Childcare Center, Restaurants, and Food Trucks Reviewed

NACOGDOCHES — The latest round of health inspections for the week of Jan. 9, 2026, included a mix of brick‑and‑mortar establishments, childcare facilities, and mobile food units across the city. Most locations received low or no demerits, with several food trucks earning perfect scores.

Jumping Jack’s Childcare — 124 Creekbend Boulevard

1 demerit
Inspectors noted the need for accurate, visible thermometers in one refrigerator and one freezer. The issue was corrected on site.

Taqueria Carlos — 107 North Street

3 demerits
Violations included missing use‑by dates and the need to keep the bases of freezers and refrigerators clean, wiped down, and sanitary.

Tomé Express — 5015 North Street, Suite B

1 demerit
Inspectors cited several wall areas near the office that were not smooth or easily cleanable.

Shali’s Indian Kitchen on Wheels (Food Truck) — Fredonia Brewery

0 demerits
The mobile unit was found 100% compliant. All commissary operations, paperwork, log books, and receipts were current and in order.

Reed’s Fish and More (Food Truck) — Nine Flags Event

0 demerits
The food truck received a perfect score, with all commissary and documentation requirements fully compliant.

Grumps Shaved Ice Truck — Nacogdoches Farmers Market

0 demerits
Inspectors reported full compliance, including up‑to‑date paperwork and log books.

Texas Pro Bowl Grill — 3801 North Street, Suite 11, Northview Plaza

5 demerits
Violations included unlabeled spray bottles containing blue and yellow solutions, which were corrected on site. Inspectors also noted missing use‑by dates, also corrected during the visit.

Headliners Bar — 3801 North Street, Suite 10, Northview Plaza

1 demerit
The bar was cited for several damaged wall and floor areas, along with water‑damaged ceiling tiles.
