Health Inspection Report Week of 2/17/2025

The Inspection Services Department carries out the general health & safety inspections of occupancies requiring a health permit within the jurisdiction of the City of Nacogdoches. The primary function of the Health Inspector is the interpretation, inspection and field compliance efforts of the technical codes and laws. These codes and laws are Federal, State and Local. In addition the Health Inspector with support from the Inspection Services Department carries out the responsibility of data entry, account billing and record keeping.

Nacogdoches Health Department Releases Latest Inspection Scores

The Nacogdoches health inspector, Ke’, issued a new round of food‑service and lodging inspection reports, with demerits ranging from 0 to 16 across restaurants, convenience stores, and institutional kitchens. Several violations were corrected on site, while others will require follow‑up.

Denny’s — 2615 North Stallings Drive

3 demerits

Denny’s was cited for lacking an accurate thermometer in the walk‑in freezer and for a prep food scoop that required re‑cleaning and sanitizing. The scoop issue was corrected during the inspection.

Citation: “PROVIDE ACCURATE VISIBLE THERMOMETER FOR WALK IN FREEZER… RECLEAN AND RESANITIZE ON PREP FOOD SCOOP.”

Lucky Stop #1 — 1218 Douglass Road

15 demerits

Lucky Stop received one of the highest demerit totals this cycle. Violations included open dumpster lids, scattered trash, improper motor‑oil storage, missing use‑by dates, unlabeled self‑serve foods, and multiple discarded pre‑packaged items. Inspectors also noted the need for thermometers in all coolers and freezers, proper scoop storage, vent cleaning, and replacement of water‑damaged ceiling tiles.

Citation: “KEEP DUMPSTER LIDS CLOSED… USE BY DATES NEEDED… DISCARDED NUMEROUS PRE PACKAGED FOOD PRODUCTS.”

Empire Donuts — 2519 Woden Road

4 demerits

Empire Donuts was cited for an unlabeled spray bottle containing blue solution—corrected on site—and for needing to clean vents and shield guards.

Citation: “RELABEL SPRAY BOTTLE WITH BLUE SOLUTION… KEEP ALL VENTS CLEAN.”

Skate‑O‑Rama — 1208 Douglass Road

0 demerits

Skate‑O‑Rama earned a perfect score with no violations.

Citation: “0 DEMERITS… 100 PERCENT COMPLIANT.”

KJ’s Convenience Store — 4712 South Street

10 demerits

Violations included open dumpster lids, damaged bathroom floors, improper ice‑scoop storage, damaged flooring and countertops, dirty vents, and unclean shelves. Several pre‑packaged food items were discarded on site, and inspectors noted missing labels and use‑by dates on some products.

Citation: “DISCARDED SEVERAL PRE PACKAGED FOOD PRODUCTS… ALL SELF SERVE PRE PACKAGED FOODS NEED PROPER LABELING.”

Kim’s Grill — 4712 South Street

11 demerits

Inspectors discarded two spatulas and sliced ham during the visit. Additional violations included missing use‑by dates, lack of soap at the handwashing sink, dirty cooler backs, missing thermometers, and uncovered foods in coolers.

Citation: “DISCARDED TWO SPATULAS… PROVIDE ACCURATE VISIBLE THERMOMETER FOR WALK IN COOLER.”

Holiday Inn — 200 Holiday Lane

1 demerit

The only violation was an unlabeled spray bottle containing blue solution, which was corrected on site.

Citation: “ONE SPRAY BOTTLE WITH BLUE SOLUTION NEEDS PROPER LABELING.”

Nacogdoches County Jail Kitchen — 2306 Douglass Road

7 demerits

One food product was discarded on site. Inspectors also cited improper food storage in the outside walk‑in cooler, lack of soap at the handwashing sink, dirty vents, and several missing ceiling tiles.

Citation: “DISCARDED ONE FOOD PRODUCT IN COOLER… SEVERAL MISSING CEILING TILES.”

Nick’s #5 — 3003 Durst Road

16 demerits

Nick’s #5 received the highest number of demerits in this inspection cycle. Violations included missing test strips, missing use‑by dates, discarded pre‑packaged foods, lack of paper towels at the handwashing sink, a non‑working ice‑cream shake cabinet, damaged floor and ceiling tiles, a missing outlet back plate, lack of a temperature log book, missing thermometers, a roof leak, and a dirty outside ice bin.

Citation: “NEED TEST STRIPS… DISCARDED SEVERAL PRE PACKAGED FOOD PRODUCTS… REPAIR ROOF LEAK.”
